According to the New Jersey State Immunization regulations, students entering 6th grade must be vaccinated prior to the start of the school year. This form must be completed and signed by your physician and can either be mailed or dropped off to the school office in an envelope addressed “School Nurse” prior to the start of school in September.
Students who have an 11th birthday occurring after September 1st will be given a two week grace period after their birthday to complete this requirement.
Students will not be permitted to attend school in September if the state requirements have not been met.
