AMS Medication Drop Off Information for the 2024-2025 School Year

Dear AMS Families,

I hope this email finds you well and you are enjoying your summer break!  Before we know it the 2024-2025 school year will be upon us and I wanted to send a quick email in regards to medications, forms, and drop-off dates. 

All students who are prescribed medications should provide the medications and appropriate completed form(s) to me prior to the start of the school year. Medication types include:

  • Daily medications your child may take during the school day

  • Emergency medications for specific health indications (ie asthma, allergies)

  • Over the counter medications with a physician’s order

You can find the following medication forms on the school website

  • Asthma Treatment Plan

  • Allergy and Anaphylaxis Emergency Plan

  • Medication Request form

These forms can be printed and brought to your child’s health care provider prior to the start of school. All medication orders should be dated after July 1, 2024.  Please note that any medication held in the health office, even over the counter medication, will require a physician’s order.  Orders for inhalers need to be on the Asthma Treatment Plan and orders for Epinephrine and/or Benadryl need to be on the Allergy and Anaphylaxis Emergency Plan.  All other medications, whether daily or as needed, can be written by the physician on the medication request form.  If you are having trouble locating the forms, please email me directly. 

Additionally, if you have an updated physical exam form for your child, you can also send that to me electronically or drop it off to my office.  This helps to ensure that I can stay on top of any changes and ensure a healthy and safe school year.

I will be in the AMS health office to receive medications and forms on:

  • August 27, 2024  9am-2pm 

  • August 28, 2024 10am-4pm

  • August 29, 2024 9am-2pm

No appointment is necessary on those days.

If you are unable to make any of the times listed above, please feel free to contact me via email at khujber@htps.us  I will be happy to find a mutually convenient time for you to make the drop off.  

Medications must be brought in by a parent or guardian, must have the accompanying doctor’s orders, and be clearly labeled with your child’s name. Please do not send medication to school with your child. If you would like to schedule a phone conference during one of these days to go over your child’s specific health needs please email me and we will set up a time.
